Long-Term Skilled Nursing Care
Holistic in focus and dedicated to innovation, Moose Lake Village provides 24-hour skilled nursing care for those with advanced health care needs. The compassionate care team includes physicians, nurse practitioners, registered and licensed nurses, nursing assistants, therapists, social workers and chaplain who work together with a deep commitment to high-quality clinical and personal care.
Skilled nursing at Moose Lake Village is based on the belief that mind, body and spirit are intricately connected. Life-enriching activities, therapeutic recreation and spiritual life opportunities are an integral part of life here.
Our Health Center
Our welcoming, home-like center features comfortable rooms, community spaces and a full range of highly-skilled nursing care. Services include long-term care, short-term transitional care, hospice and spiritual care. We also provide special care for individuals with memory impairments who also require 24-hour skilled nursing care.
Moose Lake Village has received a Four Star rating from the Center for Medicare and Medicaid Services (CMS).

Skilled Nursing Memory Care
This separate, entrance-controlled area within the Moose Lake Village health center offers compassionate care for people at all stages of Alzheimer’s disease and other memory loss conditions who also require 24-hour skilled nursing care.
Our specially-trained memory care staff work to learn each person’s life story and help them celebrate that story. Staff engage residents through individualized, purpose-filled activities to encourage residents to retain life skills longer and experience successes. Moose Lake Village memory care program is designed to emphasize abilities and celebrate moments of connection.

Hospice Care
Hospice care encircles you and your family with comfort, pain management, honesty and respect for your decisions.
Highly-specialized hospice care is provided by Moose Lake Village and hospice care partners, who bring physical, emotional and spiritual care for people with life-limiting illnesses and support for their loved ones. Your family can hand over the worries of daily physical care, allowing all of you to focus on this precious time together.
The chaplains are available for prayer, Bible reading, guidance and fostering connections with God and family. Chaplains are always respectful of personal and cultural preferences.
Spiritual Care
Moose Lake Village offers many opportunities for a rich and full life, including spiritual care. Our staff chaplain is available for the spiritual needs of residents, families and staff. All are invited to non-denominational worship services. Bible studies and conversations with our chaplains are regularly scheduled and open to everyone.
